Consent Form
From time to time we may require you to complete and send in a Consent Form. This may be as a result of your child taking part in a school excursion, activity or event. Your child will need to return this form in order to take part in extra-curricular activities.

Absentee Form
If your child/children are absent from school or you will be notifying us of future absence you are requred to fill in an absent notice through COMPASS please click on this link https://stjboorowa-nsw.compass.education/login.aspx?sessionstate=disabled for the Compass log in.
Change of Details
Please help us keep up to date by completing this form if your details have changed recently. The school office may contact you to verify details.
